Position is on site 4 days/wk, 1 day remote. We are looking for Therapists that are interested and/or experienced in implementing an Integrative Care Model. The Integrative Care Model is designed to break down silos between disciplines. Instead of treating mental health, physical health, and social needs separately, care is coordinated and collaborative, within the same clinic. Some things you may do while serving Saban patients: Behavioral Health Integration: You may provide brief interventions, crisis support, or therapy within the primary care setting. Care Coordination: You help patients access housing, food, transportation, and other social determinants of health. Team Collaboration: You participate in regular case conferences and treatment planning with medical and behavioral health staff. Some ways your role will be integrated with our Primary Care Medical team: Warm handoffs: A PCP might introduce a patient to you during a visit if behavioral or social issues arise. Shared records: Everyone on the care team can access the same patient information to ensure continuity. Screenings: Patients are routinely screened for depression, anxiety, substance use, and social needs. Whole-person care plans: Treatment plans address both medical and psychosocial needs. Overall Job Duties: Follows a population based care philosophy, ensuring accessibility to behavioral services for all referred patients emphasizing prevention, early intervention, and chronic disease management Demonstrates flexibility and openness to new ideas, adjusting care approaches based on team input, evolving patient needs, and clinic priorities. Thrives in dynamic, high-volume settings and continuously adapts workflow to support integrated care delivery. Consults with medical staff, case managers and paraprofessionals to ensure clinical and case management needs and service quality are being met; confers regarding additional identified services as needed. Maintains a working knowledge of other community mental health services and resources; effectively matches services and resource needs of clients. Collaborates, coordinates and advocates with other community professionals, servicing agencies, clients and families to ensure service plan implementation and/or linkage services; monitors linkage services to ensure that referrals are provided promptly and effectively.

  • Maintains a working knowledge of other community mental health services and resources; effectively matches services and resource needs of clients.
  • Collaborates, coordinates and advocates with other community professionals, servicing agencies, clients and families to ensure service plan implementation and/or linkage services; monitors linkage services to ensure that referrals are provided promptly and effectively.
  • An LCSW, LMFT or LPCC issued by the State of California
  • Experience implementing brief interventions ideally within a community clinic, FQHC, or integrated primary-care setting preferred.
  • Working knowledge of child protection regulations and standards
  • Track record of independent social work and capacity for developing job competency with supervision and in-service training.
  • Working knowledge of social services resources in the Los Angeles area, and related reporting procedures.
  • Demonstrated ability to respond effectively to crisis situations, and a working knowledge of crisis management technique and suicide intervention
  • Effective communication skills both oral and written, including ability to make presentations
  • Ability to be flexible and work in a changing environment.
  • Computer skills sufficient to meet production demands of the job including Microsoft Office and Clinic software for case notes.
  • Sensitivity to the different cultures represented among the clients, families and staff
  • Ability to work varied hours including evenings and weekends when necessary.
  • Bilingual skills in Spanish/English (preferred but not required)
