About The Position

JOB SUMMARY: Assists Solheim Senior Community (“Solheim”) to the success of Solheim by effectively providing nursing care and services for each resident, according to the comprehensive plan of care and under the direction of the primary care physician. The LVN will work in cooperation with the team and all departments to ensure the provision of quality care and to ensure the highest degree of resident and family satisfaction in support of the Mission and Vision of Solheim. ESSENTIAL JOB FUNCTIONS Administer Medications and Treatments: Receives and transcribes physician’s orders on the physician order sheet and the medication administration record (MAR). Ensures each medication order includes a diagnosis and it is written and/or re-capped as per policy and procedure, including any vital sign or “fyi” monitoring notes. Follows telephone order procedures. Documents physician contact as per protocol. Communicates new medication orders to the team and ensures physician’s orders are carried out. Orders new and refill medications. Verifies correct process for insurance carriers and identifies the correct pharmacy for each resident. Processes mail order refills as directed. Work with resident, family/responsible party to obtain over the counter medication. Communicate “pick-up” requests to the Van Drivers. Follows hand-washing infection prevention procedures and proper use of personal protective equipment. Maintains organization and cleanliness of the medication cart. Responsible for the correct identification of each resident and the safe, timely and accurate administration of medications, as instructed in the physician’s orders. Follows policy and procedure related to correct protocols for medication administration. Maintains compliance with “sharps safety” procedures. Ensures medications are appropriately labeled and dated with open/expiration dates per policy and procedure. Is required to complete charting of medications immediately after administration using the “Pour-Pass-Chart” method. Responsible for assessment, monitoring and effective pain management for each resident. Will document pain rating at least once per shift. Report concerns to the physician. Responsible for the assessment, monitoring and management of residents with behavioral health concerns. Obtain appropriate consent as per policy. Monitor for and document manifested behaviors, response to medication and note the presence or absence of side effects or adverse drug effects. Report concerns to the physician/psychiatrist/psychologist as needed. Document attempts at behavioral interventions prior to prn medication use. Responsible for following correct protocol in the administration and handling of cardiac medications, blood-thinning medications and insulin. Responsible for maintaining medication surveillance, safety and security at all times. Keys are to be kept “on the body” at all times and personally passed to the next shift during report. Follows policy and procedures for the handling of narcotics and other specifically FDA identified controlled drugs. On-coming and off-going licensed nurses are required to perform a narcotic count. The off-going shift may not leave until the count is deemed correct and signed off by two licensed nurses. Maintain an orderly and sanitary medication storage room environment. Store medications per manufacture instructions and according to policy and procedure. Ensure that expired medications are removed and discarded timely. Ensures emergency equipment and e-kits are current and secured. Maintain all required logs for glucose monitoring equipment, kits and refrigerator temperatures. Responsible for the discard of discontinued and expired medications. Performs physician ordered clinical treatments/procedures/tasks as assigned and according to community standard of practice, licensure, and SLH policy and procedure. Tasks may include, but are not limited to NG/GT feedings and care, wound care, ordered treatments, clinical measurements, Foley catheters, HHN treatments/oxygen administration, suctioning, ostomy care, etc. Documents according to policy and procedure. Delivery of Care: Delivers and supervises resident care, based on physician orders and the comprehensive care plan assuring that appropriate, individualized and customized quality of care is provided to all Residents. Receives and transcribes physician orders as per policy and procedure. Communicates with all disciplines to ensure care is carried out as ordered/planned. Assist and report resident status to visiting primary physicians, consulting physicians and other healthcare providers. Arrange for all in-house care from outside providers to ensure resident needs are met and documented. Ensure that visits to outside medical consultants/providers are coordinated and documented. Assist in providing telehealth services. Gives and receives report during change of shift. Participates in the coordination of shift staffing as needed. Communicate concerns to the appropriate supervisor. Monitors ADL care given by the nursing aide staff; provides support and instructions or changes in the plan of care and documentation needs. Monitors residents for any change in condition; provides urgent/first aid care and reports to MD for action. Notifies resident/family/responsible party for any and all changes. Completes License Nurse Observation and Skin Report forms; provides for 72-hour monitoring and completes documentation according to policy. Implements resulting changes to care and documents in the resident care plan. Is aware of Advance Directive and End of Life resident instructions and related physician orders. Verifies instructions when in contact with resident/family/responsible party/physician. Responsible for accessing the EMS system as needed. Monitors and provides for resident safety needs, concerns and issues. Supports a restraint free environment. Adheres to abuse prevention policy and procedures. Follows State Mandated Reporting requirements. Reports concerns to proper department and/or Administration. Complies with infection control and prevention policy and procedures. Participate in admissions, transfers and discharge procedures as assigned. Reports all issues, concerns and resident needs to the physician according to policy. Responsible for complete, accurate and timely documentation and other clinical communications as required and assigned. This includes, but is not limited to, clinical forms, progress notes, clinical summaries, care planning, medication administration records, treatment records, and clinical communication with other departments, pharmacy, lab, x-ray, physician and other ancillary health providers. Also responsible for computerized documentation. Demonstrate appropriate telephone, email, computer and fax etiquette regarding proper use and maintenance of confidentiality. Maintain expected levels of quality performance. Utilize resources. Responsible for timely follow up on consultant recommendations. Resident/Family/Guest /Staff Relations: Report issues of security, health and/or safety to appropriate supervisor as soon as practicable. Ensures resident rights are preserved. Adhere to confidentiality, privacy and dignity expectations, including HIPAA rules. Maintains effective listening and communication skills with residents, family, co-workers, physicians, ancillary and clinical service providers and vendors. Attend to residents, family and responsible parties in a responsible and pro-active manner, addressing concerns and requests in a timely manner. Utilize active listening skills, positive body language and demonstrate a positive attitude. Demonstrate good judgment, initiative, and problem-solving skills. Effectively partner and communicate issues, needs and concerns to Supervisors; appropriately utilize the chain of command. Immediately report concerns related to Theft or Loss through the appropriate chain of command. Regulatory Compliance: Follows established policies, procedures. Understands and complies with all regulatory standards set forth by governing entities. Maintains Resident/Family confidentiality and privacy in accordance with HIPPA regulations, adheres to the Compliance Plan and Solheim’s policies and procedures. Administrative Compliance: Adheres to payroll policies and properly uses timekeeping system with minimal manual changes. Maintains regular and consistent attendance. Maintains compliance with employee handbook rules. Adheres to Compliance Plan and HIPAA regulations. Maintains compliance with MSDS, personal protective equipment use and Solheim safety procedures. Performs other duties as assigned.

Requirements

  • High school graduate or equivalent.
  • Graduate of an accredited nursing school and currently licensed as an LVN in California
  • Minimum of 1 to 2 years’ experience preferred
  • Desire and ability to work with ill, aged and disabled persons.
  • Willingness and ability to read, write, speak, understand English and have the communications skills necessary to provide accurate information to residents and staff.
  • Willingness and ability to follow written and verbal direction in English.
  • Willingness and ability to maintain appropriate level of confidentiality and privacy.
