Job Highlights: This Opportunity: The primary responsibility of the Licensed Practical Nurse is to provide medical services and active treatment to individuals, under the supervision of a Registered Nurse, in a designated setting in an effort to enhance daily living. This person will provide nursing services including administration and monitoring of medication, nursing assessments and interventions.

Requirements

  • Associates: Nursing, High School Diploma (Required)
  • Drivers License (Valid) - USA, Licensed Practical Nurse (LPN) - State Board of Nursing

Responsibilities

  • Provide routine and emergent medical care as specified by State Regulations and service definitions.
  • Administer medications to individuals in a residential setting and review all staff certified in medication administration a minimum of quarterly in order to insure best practices are followed.
  • Schedule necessary physician appointments and monitor ongoing scheduling of medical appointments for individuals receiving services to ensure proper documentation is completed following such appointments.
  • Update monthly Medication Administration Records ensuring all medications are properly transcribed onto a form and maintain them in accordance with program standards and individual needs.
  • Ensure quarterly and six-month physician order updates occur for prescribed and over-the-counter medications.
  • Coordinate efforts between pharmacies, physicians, laboratories, etc., in order to ensure medical needs of individuals being supported are met and coordinated safely.
  • Conduct routine medication counts in order to ensure accurate counts have been documented and that no medication counting errors have occurred.
  • Develop and monitor programs to help individuals being supported perform self-administration.
  • Participate in preparing annual compliance surveys conducted by the Division of Facility Services.
  • Maintain an adequate supply of medical supplies.
  • Adhere to person-centered activity schedules as assigned.
  • Ensure a safe environment for the individuals being supported and ensure OSHA compliance is maintained.
  • Assist individuals in the execution of their rights.
  • Promote independence in all areas of activities of daily living.
  • Drive and travel, arrange for, or provide transportation to participants as required.
  • Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
  • Demonstrate knowledge of and comply with all agency policies and procedures, as well as service definitions related to specific program areas.
  • Complete all other relevant responsibilities as assigned by the supervisor.
