About The Position

The LEAP Program Navigator is responsible for ensuring AFC youth across programs are informed and connected to LEAP programming and will support program participants with individual life skills or workforce development needs as needed. The navigator facilitate groups, coordinate events, and support the working relationship between clients and volunteers.

Requirements

  • Experience working with adolescent/young adult population required.
  • Knowledge of psychosocial needs of LGBTQ/homeless youth population is essential.
  • Acquaintance with the local employment market, education systems, workforce programs and community resources.
  • Accuracy/attention to detail and ability to multitask and prioritize in a fast paced environment a must.
  • Ability to work independently and in teams.
  • Great interpersonal and communication skills.
  • Must be able to work nights and weekends.
  • Computer literacy, including knowledge of basic software applications (Google suite).
  • Familiarity with the internet and email communications.
  • Enters data and produces reports using agency systems with accuracy and attention to detail.

Responsibilities

  • Facilitate and support life skills and workforce development group programming for AFC youth at Ali’s Place, Emergency and Transitional Housing programs
  • Track daily participation and record in AWARDS through progress notes
  • Meet individually with AFC youth to address life skills and workforce development needs and goals, provide appropriate referrals as needed
  • Developing Individual Service Strategy with each client to address life skills, employment, educational and vocational needs
  • Coordinate with care teams to assist clients in obtaining identification, mental health care, medical care, stable housing and make appropriate referrals
  • Facilitate Standards of Care/Best Practices training, as needed, to ensure partners and volunteers are prepared to host and support AFC youth
  • Provide relevant LEAP updates in weekly team and staff meetings
  • Attend weekly meetings and supervision
  • Provide coverage and support at Ali’s Place as needed
  • Some travel may be required
  • Other duties as assigned
