About The Position

Promotes wellness and disease prevention through education, special programs, and assessments

Requirements

• Bachelors Degree or equivalent combination of education and related experience.
• 5-7 years of relevant experience and progressive experience where team leadership was demonstrated, required.

Responsibilities

• Works with patients to develop self-help plans and ensures that patients adhere to their established goals.
• Coaches patients through individualized health plans by continuously encouraging healthy habits.
• Reports on patient health status and progress.
• Develops and conducts wellness training courses.
• Ensures that wellness programs meet customer needs and satisfaction.
• Uses knowledge and skills to contribute to the development of functional objectives and principles to achieve goals in innovative ways.
• Problems addressed are of diverse scope and complexity ranging from moderate to substantial.
• Performs related duties as required.
