About The Position

The Lead RNAC (Registered Nurse Assessment Coordinator) at Chestnut Hill Lodge Health and Rehabilitation Center is responsible for overseeing the RNAC Office, ensuring accurate and timely assessments of residents' health and functional status, and managing the RAI process. This role requires a detail-oriented professional with experience in MDS completion and clinical resource utilization, who can lead interdisciplinary teams effectively in a fast-paced environment.

Requirements

  • Current Registered Nurse license from Pennsylvania.
  • Two to three years of experience as an RNAC in a healthcare setting, preferably in long-term care.
  • Experience using PointClickCare or similar healthcare information systems.
  • Proficiency in Microsoft Office applications (Outlook, Excel, Word).
  • Excellent interpersonal and organizational skills.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) is a plus.
  • Ability to work flexible hours to support business requirements.

Responsibilities

  • Overseeing the RNAC Office including staff management and organization.
  • Ensuring timely, accurate, and complete assessment of residents' health and functional status during the assessment period.
  • Completing all Medicare/Medicaid case-mix documents for appropriate reimbursement.
  • Collaborating with the Director of Rehab for appropriate assessment reference dates for Medicare/Managed Care Assessments.
  • Performing concurrent MDS reviews to achieve appropriate RUGS categories.
  • Identifying opportunities to enhance reimbursement.
  • Maintaining an accurate schedule of all MDS assessments and ensuring timely completion.
  • Modifying and inactivating assessments in accordance with CMS Correction Policy.
  • Conducting regular audits of the MDS process and validating coding documentation.
  • Ensuring timely electronic submission of all Minimum Data Sets.
  • Communicating with the Business Office and Administrator regarding RUG distribution and reimbursement impact.
  • Participating in month-end Triple Check and other relevant meetings.
  • Functioning as an RAI and Clinical Reimbursement resource for facility staff.
