About The Position

As a Registered Nurse (RN) at Summit Health, you will be an integral part of a multidisciplinary care team, providing individualized team-based care under the direction of the Primary Care Provider, Clinical Practice Manager, and Office Supervisor. This role involves delivering high-quality patient care, coordinating care activities, and ensuring effective communication among patients, staff, and physicians. The RN will apply nursing processes to assess, plan, implement, and evaluate patient care, while also providing leadership and clinical expertise in accordance with professional nursing standards.

Requirements

  • Associates degree required; Bachelor's degree preferred.
  • New Jersey State Nursing License and BLS certification required.
  • 0 - 1 years experience; 2-4 years experience preferred.
  • Ability to communicate in English, both orally and in writing.
  • Strong interpersonal and organizational skills.
  • Ability to perform diverse work assignments with time limitations with a high degree of accuracy.
  • Ability to use problem solving, critical thinking and priority setting skills.
  • Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  • Experience with Standard Office Technology in a Windows-based environment preferred.
  • Experience with patient care equipment preferred.
  • Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred.

Responsibilities

  • Assist the physician and staff with the daily coordination of patient care and physician access.
  • Function as the liaison between patients, staff, and physicians.
  • Maintain efficient patient flow.
  • Deliver direct patient care within the scope of licensure and in accordance with SMG policy.
  • Collect lab specimens according to departmental guidelines and accurately label specimens.
  • Perform Point of Care testing, EKG's, and phlebotomy as applicable.
  • Prioritize patient care by implementing physician orders and nursing procedures.
  • Ensure proper preparation for patient visits and prepare pertinent information needed.
  • Manage Nurse Visit schedule and perform patient intake, check-in/check-out as needed.
  • Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager appropriately and promptly.
  • Assure all patients are correctly identified prior to providing and/or documenting care.
  • Employ appropriate and timely use of Tasking in EHR.
  • Manage Clinical Nurse/Assigned inbox and obtain vital signs accurately and completely as required.
  • Document appropriately in EHR and administer injections according to appropriate protocol.
  • Perform patient outreach to satisfy quality measures (BP checks, vaccinations).
  • Coordinate and facilitate daily huddles and demonstrate telephone triage protocols.
  • Schedule appointments appropriately and accurately according to department guidelines.
  • Monitor daily hospital discharge list and collaborate with Care Management for TCM appropriate visits.
  • Adhere to universal safety precautions when administering medications and disposing of medical waste.
  • Review and update medication list in EHR and demonstrate understanding of prescription control and refill procedures.
  • Initiate and follow through with prior authorization requests, DME orders, and home health referrals.
  • Provide referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
  • Check and maintain logs for emergency equipment according to policy.
  • Facilitate transition of care to UCC/Hospital and maintain proficiency with department-specific equipment.
