About The Position

The Lead Placement Support Specialist is responsible for coordinating operations and workflows related to case management activities in support of specialty programs, such as Foster Care. This role involves conducting assessments to identify individual needs, assessing and identifying appropriate foster home placements for children, and developing and maintaining strong relationships with foster families. The specialist will collaborate with case managers and other team members, maintain accurate records, conduct home visits, facilitate pre-placement visits, and provide regular follow-up support to ensure stable placements. Additionally, the specialist will assist in the recruitment, training, and retention of foster families, develop comprehensive care plans, and coordinate with various state agencies to meet the needs of members. This position requires travel within assigned DSS Regions and allows for virtual work when not in the field.

Requirements

  • Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina.
  • Requires a MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.
  • Requires seven (7) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.

  • Experience working with Children, Youth, and Families served by Local Departments of Social Services.
  • Experience serving children and youth involved in Foster Care and Social Services.
  • Prior experience in managing the delivery of services to meet the needs of children and youth with complex medical conditions.
  • Knowledge of resources, support, services, and opportunities for safe community living for populations receiving in-reach and placement services.
  • Certified Case Manager Certification.

Responsibilities

  • Conduct assessments to identify individual needs.
  • Assess and identify appropriate foster home placements for children coming into care.
  • Develop and maintain strong relationships with foster families.
  • Collaborate with case managers and other team members.
  • Maintain accurate and up-to-date records of placements and communications.
  • Conduct home visits and assessments to ensure the suitability of foster homes.
  • Facilitate pre-placement visits and introductions.
  • Provide regular follow-up and support to foster families and children.
  • Assist in the recruitment, training, and retention of foster families.
  • Participate in team meetings and contribute to the continuous improvement of the foster care program.
  • Develop comprehensive care plans to address objectives and goals.
  • Coordinate with internal and external resources to meet identified needs.
  • Build meaningful partnerships with designated populations and their families.
  • Evaluate health needs and identify applicable services and resources.
  • Facilitate health rounds with other clinical centered professionals.
  • May lead and work on specialty projects and initiatives.
  • Assist with process improvements activities.
  • Train and mentor staff.
  • Perform case/chart audits and ensure departmental compliance.
