Requirements

Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school.
Texas pharmacy licensure or eligibility.
A minimum of 3-5 years of clinical pharmacy experience, with a focus on protocol development preferred.
3-5 years of management experience preferred
Strong knowledge of pharmacotherapy, clinical guidelines, and quality improvement methodologies.
Excellent analytical, organizational, and communication skills.
Ability to work collaboratively in a multidisciplinary team environment.
Proficiency in electronic health record (EHR) systems and pharmacy management software.

Nice To Haves

Board of Pharmacy Specialties (BCPS) certification preferred
PGY-1 residency or equivalent experience preferred

Responsibilities

Lead a team of pharmacists who design and develop clinical protocols for medical condition-specific medication management programs such as, but not limited to, Congestive Heart Failure, COPD, diabetes, hypertension, etc.
Responsible for hiring, onboarding, and the development of a team of pharmacy personnel.
Collaborate with multidisciplinary teams to ensure protocols align with guideline driven medication therapy, and other established clinical guidelines and best practices.
Provide training and support to pharmacy staff and other healthcare professionals on the implementation of new protocols.
Supervise the development of educational materials and resources for ongoing staff development.
Partner with the Compliance and Quality teams to monitor and evaluate the effectiveness of clinical protocols and make recommendations for improvements.
Participate in quality assurance initiatives to enhance medication safety and efficacy.
Engage in direct patient care activities as needed, including medication reconciliation and therapeutic monitoring.
Collaborate with healthcare providers to optimize pharmacotherapy for patients.
Monitor and uphold adherence to evidence-based clinical guidelines and gold-standard treatment protocols across disease states.
Ensure all protocols comply with regulatory and institutional policies and standards.
Assist in preparing documentation for audits and regulatory reviews.
