About The Position

The LPN Lead is responsible for assisting the physician and staff with the daily coordination of patient care and physician access, functioning as the liaison between patients, staff, and physicians. This role involves providing professional nursing care to office patients in noncomplex situations under the supervision of a Physician and Clinical Practice Manager or Clinical Coordinator. The LPN Lead delivers direct patient care within the scope of licensure and in accordance with SMG policy, assuming accountability for the quality of care. Requirements

  • Vocational / Technical School / Diploma Program required.
  • New Jersey State Nursing License required.
  • BLS certification required.
  • 0 – 1 years experience.
  • Ability to communicate in English, both orally and in writing.
  • Strong interpersonal and organizational skills.
  • Ability to prioritize and organize multiple tasks in a timely manner.

Nice To Haves

  • Associates degree preferred.
  • 2-4 years experience preferred.
  • Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  • Experience with Standard Office Technology in a Window based environment preferred.
  • Experience with patient care equipment preferred.
  • Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred.

Responsibilities

  • Providing professional nursing care to the office patients in noncomplex situations under the supervision of Physician and Clinical Practice Manager or Clinical Coordinator.
  • Delivering direct patient care within the scope of licensure and in accordance with SMG policy.
  • Assuming accountability for the quality of care, which he/she has delivered including maintaining standard precautions.
  • Collecting lab specimens according to departmental guidelines.
  • Accurately labeling specimens.
  • Prioritizing patient care by implementing physician orders and nursing procedures.
  • Documenting appropriately in the Electronic Health Record (EHR).
  • Assuring all patients are correctly identified prior to providing and/or documenting care.
  • Communicating & providing care consistent with age, cultural, spiritual and developmental needs of the patient.
  • Maintaining efficient patient flow.
  • Employing appropriate and timely use of Tasking in EHR.
  • Scheduling appointments appropriately and accurately according to department guidelines.
  • Administering injections according to appropriate protocol.
  • Documenting injections accurately in EHR.
  • Appropriately demonstrating telephone triage protocols and document in EHR.
  • Effectively communicating problems, concerns or issues to the Office Supervisor/Manager appropriately and promptly.
  • Ensuring proper preparation for patient visit.
  • Preparing pertinent information needed for patient visit.
  • Obtaining vital signs: BP, temp, pulse, height & weight accurately and completely as required.
  • Documenting appropriately in EHR.
  • Reviewing and updating medication list in EHR.
  • Demonstrating understanding of prescription control and prescription refill procedures.
  • Accurately and completely documenting known allergies in EHR.
  • Appropriately entering orders and scheduling appointments in the Practice Management System (PMS) and EHR.
  • Providing referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
  • Appropriately adhering to universal safety precautions when administering medications and disposal of medical waste.
  • Maintaining proficiency with department specific equipment, troubleshoots and reports malfunctions promptly.
  • Facilitating transition of care to UCC/Hospital.
  • Checking and maintaining logs for emergency equipment according to policy.
  • Assisting in ordering and maintaining of all supplies as assigned.
  • Demonstrating flexibility with various work schedules.
  • Recognizing and performing duties which need to be performed although not directly assigned.
  • Responsible for maintenance of staff and provider schedules in Shift Hound.
  • Work with Office Supervisor/Clinical Coordinator to provide adequate staff coverage.
  • Responsible for oversight of medical and office supplies and ordering of supplies.
  • Monitor employee performance of staff assigned to office.
  • Discuss concerns and issues with Office Supervisor/Clinical Coordinator/Clinical Practice Manager.
  • Responsible for addressing and resolving patient complaints/issues.
  • Communicate issues and resolution to Providers, Office Supervisor/Clinical Coordinator/Clinical Practice Manager.
  • Bring concerns expressed by providers to Office Supervisor/Clinical Practice Manager.
  • Notify Office Supervisor/Clinical Coordinator/Clinical Practice Manager of any potential health and safety hazards in the office.
  • Responsible to place and follow through on Facilities/IT work orders related to the office setting.
