About The Position

The Lead Medical Coordinator provides advanced non-medical/clerical case coordination and health services support for youth placed in ORR-funded residential shelter care. This role ensures timely access to medical, dental, behavioral health, and specialty services while maintaining compliance with ORR policies, DHUC standards, HIPAA requirements, and agency protocols. The position serves as a central liaison between the Health Center, residential staff, clinicians, external medical providers, and the multidisciplinary treatment team to support youth health outcomes within a trauma-informed, culturally responsive framework. The position requires the ability to work independently, be detailed oriented, exhibit organizational skills, be proactive, and work as a team member.

Requirements

  • Licensed Vocational Nurse (LVN)
  • Registered Nurse (RN) or higher health-related qualification (e.g. Physician Assistant, Nurse Practitioner).
  • Minimum 1 year experience in Social Services, including DMH documentation is preferred.
  • Minimum two (2) years in residential care, behavioral health, medical coordination, or child welfare settings.
  • Experience with ORR, DHCS, DMH, or DHS documentation standards strongly preferred.

  • bi-lingual skills in Spanish/English

Responsibilities

  • Computer skills sufficient to meet production demands of the job, including Microsoft Office, TIER, MyEvolve software, and knowledge of the ORR UC Portal.
  • Skill in maintaining detailed, accurate and complete records.
  • Effective oral/written communication skills; bi-lingual skills in Spanish/English preferred.
  • Strong organizational and case coordination skills.
  • Ability to manage confidential health information.
  • Sensitivity in engaging with clients, families, treatment teams, and staff without regard to race, religion, ancestry, perceived or actual sexual orientation, gender identify or expression.
  • Knowledge of emergency procedures of the agency.
  • Ability to work effectively as part of a team.
  • The incumbent must be able to perform this job safely, without endangering the health or safety of themselves or others.
