About The Position

Our Transition Foster Care Program for Unaccompanied Children (UC), overseen by the Office of Refugee and Resettlement (ORR), ensure the well-being and safety of unaccompanied children (ages 0-17) who cross the US border without an adult. Unlike traditional congregate care settings, our approach involved placing unaccompanied children in foster homes while diligently searching for, and assessing, potential sponsors identified in the US. During their stay with us, we provide medical care, case management, engaging life skills classes, and essential mental health services. Cayuga Centers is seeking a compassionate individual for the Lead Medical Coordinator role in the UC programs. The Lead Medical Coordinator will supervise the medical coordinator team to support the medical, safety, and well-being of every child in care and ensure compliance with established regulator agencies and federal standards requirements. This role will use sound medical and professional judgment to promote best practices and deliver quality care.

Requirements

  • Minimum 1 year of experience in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care - required
  • Must be Licensed Registered Nurse (RN) with a Bachelor's; or higher health-related qualification (Physician Assistant or Nurse Practitioner) - required
  • Strong communication and organizational skills - required
  • Knowledge of medical office environment, workflow, and terminology
  • Strong knowledge and experience of working with Microsoft Outlook, Excel, and Word
  • Excellent typing skills
  • Ability to multitask within a fast-paced medical office setting
  • Ability to work independently and in collaboration with agency healthcare providers and nursing support staff
  • Excellent communication and proven human relations skills
  • Bilingual - English/Spanish required. All agency documents and records are in English, so English writing proficiency is required. Position itself requires Spanish speaking, writing, and reading proficiency
  • Demonstrated ability to perform duties with minimal supervision and to exercise greater judgment
  • Ability to travel to other agency locations
  • Valid NYS Driver's License preferred
  • Word processing: Including responding to emails, composing letters, completing the computerized documentation system, etc. This involves sitting at a computer for several hours
  • Completing paperwork - such as copying, scanning, and faxing documents
  • Must be able to lift up to 50 pounds

  • Regular hours entail Monday - Friday 9:00am - 5:00pm
  • Must be flexible to meet program needs
  • Weekend and evening on-call hours - required
  • Holidays may be required

Responsibilities

  • Support and train other medical coordinators
  • Collaborate with the training department to provide relevant training on medical, dental, and public health topics, with a focus on pediatric, adolescent, and migrant health issues
  • Take lead in partnering with providers and ensure all documentation is up to date in UC portal and file for children that enter our care
  • Update and maintain accurate/up-to-date UC portal case files and records with information pertaining to nursing care, child's medical history, consult notes, treatment plans, and observations; chart all pertinent information in the UC portal case files/records in accordance with agency policy and licensing standards
  • Completion of treatment authorization requests (TAR's)
  • Completion of SIRs
  • Assembly of discharge packets
  • Working knowledge of and updating pertinent spreadsheets
  • Knowledge and experience of working with current EMR (Medent)
  • Perform other duties as requested by your immediate supervisor and program/agency leadership
