About The Position

The Lead Medical Coordinator (LMC) is responsible for guiding and ensuring the quality of medical services, treatment, and/or interventions provided to Unaccompanied Children (UC) in the program in collaboration with the contracted physicians. Lead Medical Coordinator is the key monitor of patients' health through observing and assessing their records, symptoms, and reactions to treatment and care. They often have extensive interaction with the UC as well, guiding and instructing them in aftercare measures. The LMC is devoted to the delivery of medical care with the emphasis on disease prevention and health maintenance while ensuring compliance with State licensing, Office of Refugee Resettlement (ORR), and all policies and procedures implemented by CFRC.

Requirements

  • Licensed Registered Nurse (RN) with a Bachelor’s OR Higher health-related qualification (e.g. Physician Assistant, Nurse Practitioner) required.
  • Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
  • Must be bilingual (English/Spanish) – speak, read and write.

Nice To Haves

  • Pediatric experience is preferred.

Responsibilities

  • Overseeing health services as well as coordinating the provision of timely, appropriate health services for children in care.
  • Supervise Medical Coordinators and Medical Assistants by providing appropriate staff training on medical, dental, and public health topics relevant to pediatric, adolescent, and migrant health.
  • Developing and maintaining policies and procedures for confidential health record management, triage of health concerns, medical emergency response, environmental health and safety, and post-release health care planning/care coordination.
  • Assessing, facilitating, and monitoring all components of the medical services department and staff, including QA of medical program including auditing of electronic medical records.
  • Overseeing and management of all diagnostic filing, electronic, scans and hard copy documentation to the current chart.
  • Compile, record, process, and maintain client medical records in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health services department. including but not limited to results, physician appointments and follow ups.
  • Monitor performance, identify areas for improvement and seek opportunities to enhance procedural efficiency and productivity.
  • Collaborate with the nursing team to create a plan of care for all patients, to include coordinate youths’ vaccination and vaccination records.
  • Provide primary and emergency care for injuries and illnesses.
  • Administration of over the counter and prescription medications and implementing health care provider orders perform treatments, procedures and special tests, and document treatment as directed by physicians and required by company policy and local/state/federal rules and regulations
  • Develop corrective action plans, disciplinary memos, and annual evaluations for medical department staff in collaboration with the Program Management Team and Human Resources.
  • Assist the medical providers’ staff with examination procedures when needed, such as patient screening /triage, medical procedures and other patient care.
  • Ensure billing and TARs are processed in a timely manner to cover doctor's appointments.
  • Ensure compliance with all health care regulations, including HIPAA and OSHA.
  • Responsible for reporting requirements under the program for the Health Department and maintain and implement any recommendations given by the Public Health Departments, CDC, and ORR.
  • Create strategies and action plans for the prevention of infectious disease outbreaks. Investigate possible outbreaks and coordinate proper resources in response to any confirmed outbreak. Responsible for reporting requirements under the program for the Health Department.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or provided and to ensure that changes to policy or practice are implemented and adhered to.
  • Participate in the selection, onboarding, and training process for medical services staff and the development of on-the-job training for medical staff as needed.
  • Oversee youth evacuations during inclement weather or unforeseen occurrences, and maintain a safe, clean, and hazard-free work environment.
  • Other duties, tasks which may be assigned by an appropriate supervisor or manager.
