About The Position

PHMC serves as both a direct service provider to individuals, families, and communities across the region and as an intermediary agent — managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment. JOB OVERVIEW: Lead Medical Case Manager oversees care coordination and provides a full range of support for clients diagnosed with HIV/AIDS at the Care Clinic. The Lead Medical Case Manager reports to the HIV Program Manager.

Requirements

  • One-two years of experience in HIV/AIDS case management services, or related field.
  • Bachelor’s Degree required.
  • Degree must be in Social Work, Human Services, Sociology, Psychology, Nursing, Public Health, or a related field from an accredited school.
  • Other degrees may be considered if the candidate has Medical Case Management experience servicing people living with HIV/AIDS
  • Strong interpersonal skills.
  • Strong organizational skills.

  • Bilingual/bi-cultural a plus.
  • Knowledge of community resources helpful.

Responsibilities

  • Maintain a caseload of people living with HIV/AIDS.
  • Provide oversight of Medical Case Managers (MCMs) and conduct patient chart reviews.
  • Conduct MCMs monthly supervisions.
  • Complete a biopsychosocial assessment and service care goals with each client at the time of intake.
  • Monitor and update service care goals and assessments quarterly in coordination with the interdisciplinary team.
  • Collaborate with an interdisciplinary team in the evaluation of patient needs and treatment progress.
  • Maintain contact with clients based on program need, client need, and service care goals.
  • Documentation of adherence to medical appointments and medical care plan, including HIV medication adherence.
  • Provide trauma-informed therapeutic support in collaboration with an interdisciplinary team, including crises.
  • Identify appropriate resources and external support systems to address client needs, including advocating and negotiating linkages to resources.
  • Available to provide home visits and accompany clients to medical appointments and outside treatment facilities as necessary.
  • Maintain accurate and timely documentation of all client contacts (using D.A.P. format), forms, and applications in EHR software.
  • Coordinates and documents Psychosocial Support Groups as required by the Division of HIV Health (DHH).
  • Assist Program Manager with Quality Improvement Projects as directed by DHH.
  • Perform analysis and reporting of MCM data as required by DHH.
  • Provide real-time data support to MCM staff.
  • Support the assessment of impact and outcomes to improve clinical care and meet reporting obligations.
  • Contribute as a team member and share in the responsibilities required to maintain operations and serve the mission of PHMC. This includes, but is not limited to, obtaining training, participating in events, flexibility in scheduling, covering the commitments of coworkers when they are unable, and other duties as assigned.
