Provides direct and indirect clinical social work and discharge planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel. Frequent contact with patient care services staff, physicians, and ancillaries to discuss patient care and diagnostic needs. Participate in care rounds and other interdisciplinary collaboration within the in-patient and outpatient programs. Frequent contact with community agencies, third party payers, outside review organizations and case managers. The medical social worker/team leader, under the direction of the manager, will coordinate and oversee the activities of the social work team, providing daily coordination of team workload and assignments as well as resource to team members regarding difficult clinical and operational issues. Administrative duties include assisting in selection of and orientation of new employees, monitoring team performance, participation in leadership with manager/director.
