About The Position

General Description: The Lead Housing Case Manager will be a skilled leader, helping to oversee the case management team, meeting clients where they are, and assisting with quickly moving people into permanent housing. In this position, this staff member must maintain a positive and respectful attitude; have a strong work ethic while keeping the needs of the clients as the primary priority. In the absence of the Manager of Housing and Supportive Services, the Lead Housing Case Manager will provide direct supervision to the Housing Case Managers.

Requirements

  • Bachelor’s degree in social work or related field required, master’s in social work or related field preferred.
  • Minimum three years of clinical supervisory experience.
  • Minimum three years’ experience in social and human services field dealing with diverse populations.
  • Must have a valid driver’s license.
  • Knowledge of best practices including Housing First, Harm-Reduction, Motivational Interviewing, and Trauma Informed Care along with their methods of service delivery.
  • Advanced in entering data into databases, Excel spreadsheets, Microsoft Office suite proficiency required.
  • Ability to establish and maintain positive working relationships.
  • Must be willing to complete required training, including HMIS, Relias, Mandt, Narcan, CPR, and minor First Aid.
  • Ability to advocate for individuals experiencing homelessness

Nice To Haves

  • Bilingual in Spanish and English preferred.

Responsibilities

  • Provides emergency intervention in response to hostile situations and crisis and follows protocol to address emergencies.
  • Demonstrate flexible and efficient time management and ability to prioritize workload.
  • The Lead Housing Case Manager will have a caseload of up to 15 clients.
  • Serve as point of contact when the Manager of Housing and Supportive Services is out.
  • Support colleagues with compliance with HMIS policies and procedures.
  • Oversee bi-weekly audits of Case Management Team entries to the Coordinated Entry data in agency database to ensure compliance with data standards and record-keeping.
  • Supports other Case Management team members to make necessary corrections to information entered and compiles, sorts, and verifies accuracy of data to be entered, while keeping record of work completed.
  • Works with Manager of Housing and Supportive Services to support goals of exceeding established performance and improving client outcomes.
  • Participate in on-going training and professional development opportunities.
  • Assist with training new housing case managers as they on-board (using Case Management team training plan).
  • Oversee motivational contracts with clients and collaborate with Manager of Housing and Supportive Services to ensure they are administered in an appropriate and timely fashion.
  • Provide copies of motivational contracts to Housing Case Managers and client care and operations team to ensure they are follow-up on.
  • Facilitate CES prep meeting and filling in as needed, when another Housing Case Manager cannot be on the call.
  • Maintain calendar of work schedules for clients.
  • Organize and facilitate Life Skills and other group conversations with clients.
  • Supports overflow management during hypothermia season.
  • Other duties as assigned.
  • Advocate for individuals experiencing homelessness.
