About The Position

The Lead Home Visitor position within the Bureau of Maternal Infant and Reproductive Health at the NYC Department of Health is a vital role aimed at enhancing maternal and child health services. In this role, the Lead Home Visitor will conduct home visits to eligible mothers, delivering essential prenatal and post-partum education, health assessments, and referrals to necessary services. The responsibilities include providing maternal and infant health education on various topics such as breastfeeding, safe sleep practices, mental health, and chronic illness management. Additionally, the Lead Home Visitor will assess home environments for potential health hazards, such as lead exposure, and make appropriate referrals for follow-up care. Accurate documentation and monitoring of case data are crucial components of this position, as is the ability to connect families with program specialists and community resources. The Lead Home Visitor will also engage with community providers to identify and recruit eligible families for the program. This includes promoting the program at community events, conducting neighborhood health assessments, and supporting senior management with additional tasks as needed. The position requires strong communication skills, both written and verbal, and fluency in English and either Spanish or French is preferred. Experience in home visiting or community outreach, as well as knowledge of maternal and child health, will be beneficial for success in this role.
