About The Position

As the largest nonprofit affordable housing provider in the nation, we build, preserve, and manage program-enriched housing across the country. The Lead Healthcare Coordinator Specialist will provide leadership, oversight, and coaching to Healthcare Coordination Specialists. Develop strategies to link residents with health care and care coordination services. Develop relationships with the primary care clinics, medical providers, care coordinators, health plans and other health service providers to increase the health and wellness of residents and reduce unnecessary healthcare utilization. This is a grant-based position. This position is eligible for a hybrid work schedule. Francis Heights is affordable housing community for senior citizens in Denver.
