Core Values: TRACK Teamwork Relationship Building & Communication Accountability Customer/Client Focus Know Yourself PREFERRED EDUCATION and EXPERIENCE: High School Diploma or GED Valid driver’s license, clean driving record, and reliable transportation 2 years of experience working with individuals with disabilities Ability to engage with others using courtesy, tact, and good judgment Appropriate safety equipment and first aid kit in personal vehicle SUMMARY/OBJECTIVE: Assist and support individuals to maintain health status, develop skills in instrumental activities of daily living, adapt to community and home environments, identify and access community activities and public resources, facilitate relationship building with the greater community, and to empower persons in services to achieve their goals. Support managers with collecting required documentation, provide quality assurance support with medication and money management logs, facilitate activity schedule and planning, coordinate vehicle oversight checks, support individuals with peer-lead activities and committees. #hotjob ESSENTIAL DUTIES and RESPONSIBILITIES: Provide support to help individuals learn to be more independent and to meet their goals with variety of day-to-day events. This may include, but is not limited to, bathroom support, household chores, budgeting, making friends, exploring their community, shopping, meal prepping, advocating for themselves, eating, etc. Demonstrates effective and professional communication with individuals and their families/support systems as well as with providers serving the individuals while establishing and maintaining professional boundaries. Assist with personal care and Activities of Daily Living (ADLs), including but not limited to, directly supporting with baths, oral care, dressing, grooming, etc. Provide training in skills related to the use of community resources, such as transportation, shopping, and social and recreational activities. Monitor the health and physical condition of individuals and assist with medication management and other medical needs when applicable. Recognize and report suspected cases of abuse, neglect, or exploitation. Supervise individuals to ensure their health and safety. Provide behavioral support as appropriate and train individuals in adaptive behavior for home and community settings, such as managing anger, handling social encounters with others, and developing a support network. Facilitate the active involvement of the individuals served and their families/support systems in the service delivery process, maintain knowledge of individuals’ daily goals, and document progress toward achieving those goals. Assist individuals in handling developmental and life transitions, stay knowledgeable about available community resources, and help connect individuals with these resources. Ensure quality, timely completion, and submission of required documentation. Maintain accurate time records by clocking in and out at the start and end of each shift. Maintain current personal car insurance and registration. Provide quality assurance checks to documents including medication administration record, attendance logs, individual activity budget ledgers, personal care data sheets, and behavior support data sheets. Collect, scan, and upload documentation to individuals’ profiles monthly. Facilitate a collaborative schedule through activity development, planning, and implementation with individuals and staff. Work collaboratively with Activities Coordinator to develop preferred activities and explore opportunities for volunteering, exploration, and building relationships with the greater community. Lead safety drills; ensure first aid supplies are stocked, in date, and accessible. Maintain active supervision; adjust planning and guide staff to ensure ratio is maintained. Conduct environmental checks, maintain supply inventory, report hazards and maintenance needs to supervisor. Identify and communicate individual needs and changes to supervisor and other members of individual’s support team. Train and mentor new DSPs to ensure consistent support for individuals. Attend meetings, including ISP and SIS meetings, and provide input on services provided. Ensure company standards and practices are followed by all staff. Requirements
