About The Position

Counselors are primarily responsible for the delivery of individual, group and family counseling services and completion of documentation required for the clinical record including, but not limited to; development of the Individual Treatment/Service Plan (ITP/ISP), psychosocial assessment, daily notes, and monthly reviews. Counselors will attend interdisciplinary treatment team meetings, monitor treatment throughout the client’s care, and establish parameters for discharge planning. Counselors are responsible for crisis intervention and de-escalation of situations to ensure safety to all. This includes assessment of crisis situations to determine severity and making recommendations to peers to insure consistency and appropriateness of staff responses to issues. Counselors will develop an approved curriculum as the basis for group and individual counseling.

  • Minimum Bachelor’s Degree from an accredited college or university with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or related human services field.
  • Completion of job duties may require working before and after normal working hours.
  • Regular attendance is required.
  • Being able to work as many hours as is necessary to complete job tasks is a required and essential duty of this position.

Responsibilities

  • Provide daily supervision of BHT and LBHT staff, counselors, interns, volunteers, peer support specialists, case managers, health advocates, and food specialist staff involved in direct client interactions.
  • Ensure adherence to program policies and procedures.
  • Support Program Managers and LPNs as needed, in accordance with regulatory laws and requirements.
  • Deliver individual and group counseling services, case management, education, community outreach, and referrals to appropriate services and resources.
  • Utilize counseling and psychotherapy techniques to assess and address emotional and mental health conditions, including cognitive, behavioral, and affective disorders.
  • Maintain a secure and confidential distance delivery system, ensuring client informed consent and safety.
  • Adhere to all relevant laws, statutes, regulations, and standards for distance-based services.
  • Evaluate client needs and make informed decisions regarding the provision of individual, group, and family counseling.
  • Conduct client interviews and assessments to determine eligibility and appropriateness for program participation and referrals.
  • Implement crisis intervention strategies as needed, in accordance with WestCare procedures.
  • Provide direct counseling services and monitor client behavior.
  • Develop and implement Individual Treatment/Service Plans.
  • Report unusual incidents to the Program Manager, Program Director, Regional Vice President, or relevant regulatory agency as per the established point of contact.
  • Manage PHMIS data entry in alignment with program requirements.
  • Conduct CE assessments in compliance with regulatory guidelines.
  • Maintain an outreach log for data entry into the Clinical Data System (CDS) as per program needs.
  • Utilize and manage Electronic Health Record (EHR) systems for data entry and client information tracking, ensuring program alignment.
  • Participate in decisions regarding client referrals for ancillary services, such as family support groups and continuing care.
  • Complete clinical documentation and other reports as required.
  • Attend interdisciplinary treatment team meetings and contribute to discharge planning.
  • Maintain regular and consistent attendance to ensure continuity of client counseling services.
  • Adhere to all WestCare policies and procedures.

  • Adapt to new requirements, including potential vaccinations or other mandates issued by local, state, federal, or funding agencies, as communicated by Human Resources or designated personnel.
  • Conduct monthly quality assurance reviews of treatment case files to ensure compliance with program standards.
  • Oversee and facilitate client intake and program orientation, including psychological and social assessments, and the development, implementation, and ongoing review of Individual Treatment Plans.
  • Supervise the maintenance of client treatment files within the EHR system, including documentation of vocational and educational activities.
  • Assist with data collection, statistical reporting, and Quality Improvement reports as needed.
  • Maintain and cultivate relationships with referring and supporting agencies and represent the agency at community meetings as requested.
  • Provide coverage for supervisees during absences due to illness, training, or other reasons.
  • Plan and schedule therapeutic activities for general client supervision.
  • Prepare client progress reports as necessary.
  • Perform any other duties as assigned.
