About The Position

The Lead Community Health Worker reports to the Program Manager and serves as a senior field leader within our Enhanced Care Management and Field Medicine programs. This role blends direct patient care with structured mentorship and operational support for frontline CHWs. You will model high-quality engagement, strengthen onboarding and training, and reinforce program integrity across complex CalAIM populations. This is an opportunity to elevate care delivery while shaping the next generation of community-based health workers.

Requirements

  • High School Diploma or equivalent required
  • Valid driver’s license, automobile insurance, and reliable transportation required
  • 3+ years of experience working with CalAIM populations of focus including people experiencing homelessness, adults with SMI or SUD, adults transitioning from incarceration, and adults with complex medical needs required
  • Experience mentoring, training, or informally leading peers preferred
  • Strong understanding of care coordination workflows and documentation standards preferred
  • Ability to work independently in outdoor and community-based environments preferred
  • Excellent communication, interpersonal, and organizational skills preferred
  • Proficiency in documenting in case management software preferred

Responsibilities

  • Manage a caseload of patients with complex medical, behavioral health, housing, and social needs
  • Complete assessments, co-develop care plans with interdisciplinary teams, and execute care coordination activities aligned with ECM standards
  • Build strong rapport with patients to promote self-management, stability, and sustained behavior change
  • Serve as a field-based trainer for new CHWs, coordinating shadowing, onboarding, and structured skill development
  • Provide real-time coaching and mentorship to CHWs through joint field visits and observational feedback
  • Act as an escalation resource for complex patient situations in coordination with Program Managers
  • Support outreach and enrollment of eligible members into the Akido ECM Program
  • Ensure accurate, timely, and compliant documentation in designated case management systems
  • Model safety protocols, workflow adherence, and professional standards in dynamic field environments
  • Assist Program Managers in reinforcing quality improvement initiatives and performance expectations
