About The Position

We are seeking an experienced, adaptable Lead Caregiver (or CNA) to join our team in a dynamic, leadership-track role. The Lead Caregiver is our go-to team member for launching new cases, covering urgent shifts, supporting higher-acuity clients, and providing field support to help develop our caregiver team. This role is the first step into leadership at The Next Chapter, with an advancement path toward Assistant Care Coordinator .

Requirements

  • 2+ years caregiving experience in a home care setting required
  • 5+ years strongly preferred
  • Experience with dementia, hospice, and high-acuity clients required
  • Background check, drug screening, DMV check, and negative TB test
  • Fingerprint Clearance Card preferred
  • Current First Aid & CPR
  • Caregiving Certification or CNA license required before client assignment (Certification support available for highly experienced applicants)
  • Reliable personal vehicle, active auto insurance, valid driver’s license
  • Must be willing to cover our full service area (generally within 30 minutes from Chandler; occasional farther travel may be needed)
  • Full Monday–Friday availability for any shift ( days, evenings, or overnights )

  • CNA preferred but not required

Responsibilities

  • Assist with ADLs including bathing, grooming, dressing, toileting, feeding, and mobility.
  • Provide advanced support for clients who are bed-bound or require mobility equipment (e.g., Hoyer lifts).
  • Deliver compassionate dementia and Alzheimer’s care.
  • Provide calm, grounded support for hospice and end-of-life cases.
  • Offer meaningful social interaction and emotional support.
  • Perform light housekeeping and prepare nutritious meals to maintain a safe, clean, and comfortable environment.
  • Provide medication reminders and support consistent daily routines.
  • Transport clients to appointments and activities as needed.
  • Follow individualized care plans and observe for changes in condition.
  • Serve as the first caregiver on new cases to establish routines, expectations, and rapport.
  • Provide urgent coverage for call-outs and last-minute needs M-F.
  • Mentor caregivers in the field, reinforce best practices, and build confidence.
  • Assist leadership with hands-on quality assurance at client homes.
  • Communicate proactively with TNC leadership regarding client observations, care plan needs, and family dynamics.
  • Identify risks, challenges, or needed supplies promptly.
  • Remain steady, patient, and solution-oriented in challenging family or care scenarios.
  • Participate in occasional volunteer or community events as a representative of TNC.
