About The Position

The Lead Care Manager is responsible for coordinating and overseeing the care of patients within a healthcare setting, ensuring that all care services are delivered effectively and in alignment with patient needs. This role involves managing a team of care managers and care coordinators, liaising with healthcare providers, and ensuring that care plans are created, implemented, and updated as needed.

  • Must be knowledgeable of current word processing and spreadsheet software, Microsoft Office, internet functions and knows how to use a computer.
  • Registered Nurse (RN) with a current Louisiana State Board of Nursing license.
  • All candidates will be required to submit to a criminal background check and screening as part of preemployment.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals
  • Ability to write routine reports and correspondence
  • Ability to speak effectively before groups of customers or employees of organizations
  • Ability to define and solve practical problems, collect data, establish facts and draw valid conclusions.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form
  • Minimally, must be a Registered Nurse with 3+ years of experience in care coordination or healthcare management preferred.
  • Previous roles as a care coordinator, case manager, or in similar positions.
  • Experience with managing patient care plans, interacting with multidisciplinary teams, and handling complex cases.

Nice To Haves

  • Certification in Care Coordination (e.g., CCM) is a plus.
  • bilingual a plus.
  • Masters prepared Registered Nurse (RN) preffered.

Responsibilities

  • Care Coordination: Oversee and manage patient care plans, ensuring they are customized based on patient needs and preferences.
  • Team Leadership: Lead and supervise a team of care managers and care coordinators by providing support, guidance, and mentorship.
  • Collaboration with Providers: Work closely with physicians, nurses, and other healthcare professionals to ensure that care plans are aligned with medical recommendations.
  • Patient Advocacy: Act as an advocate for patients, addressing any concerns or issues they may have and ensuring that they receive quality care.
  • Assessment and Evaluation: Conduct patient assessments to determine care needs and make recommendations for necessary services or adjustments to care plans.
  • Education and Support: Educate patients and their families about care plans, treatment options, and available resources.
  • Documentation and Reporting: Maintain accurate records of patient care plans, updates, and progress. Ensure all documentation complies with healthcare regulations.
  • Problem Solving: Address any challenges or barriers that patients or the care team may face and work towards finding solutions.
  • Compliance and Quality Control: Ensure that care management processes comply with organizational policies, healthcare regulations, and standards of practice.
