Reporting to the Site Supervisor, the Lead Care Manager provides strategic oversight and direct service leadership for the CalAIM Enhanced Care Management (ECM) and Community Supports (CS) services. This position is responsible for overseeing daily program operations across LFCD’s Oakland and Alameda County locations, with a strong focus on serving unhoused, medically complex, and vulnerable populations. The Lead Care Manager coordinates trauma-informed, person-centered services in collaboration with managed care plans (MCPs), healthcare providers, housing agencies, and community-based organizations (CBOs). Key responsibilities include delivering direct case management, leading outreach and engagement activities, facilitating housing assessments and placements, and guiding participants through complex systems of care. This role ensures that culturally responsive practices are fully integrated into service delivery while maintaining compliance with Medi-Cal, HIPAA, and CalAIM standards. Additionally, the Lead Care Manager supports performance monitoring, continuous quality improvement, and cross-sector partnership development to advance long-term health and housing stability for program participants. This position requires travel and service coverage between LFCD’s offices in Oakland and Alameda County.
