About The Position

Reporting to the Site Supervisor, the Lead Care Manager provides strategic oversight and direct service leadership for the CalAIM Enhanced Care Management (ECM) and Community Supports (CS) services. This position is responsible for overseeing daily program operations across LFCD’s Oakland and Alameda County locations, with a strong focus on serving unhoused, medically complex, and vulnerable populations. The Lead Care Manager coordinates trauma-informed, person-centered services in collaboration with managed care plans (MCPs), healthcare providers, housing agencies, and community-based organizations (CBOs). Key responsibilities include delivering direct case management, leading outreach and engagement activities, facilitating housing assessments and placements, and guiding participants through complex systems of care. This role ensures that culturally responsive practices are fully integrated into service delivery while maintaining compliance with Medi-Cal, HIPAA, and CalAIM standards. Additionally, the Lead Care Manager supports performance monitoring, continuous quality improvement, and cross-sector partnership development to advance long-term health and housing stability for program participants. This position requires travel and service coverage between LFCD’s offices in Oakland and Alameda County.

Requirements

Bachelor's Degree required; concentration in Social Work, Psychology, Public Health, or related fields preferred. In lieu of a degree, 2+ years of direct service experience in healthcare or community-based settings is required; CalAIM program or Medi-Cal managed care experience is highly preferred.
At least 1 year's experience working with sensitive needs populations, including but not limited to low-income, unhoused, unemployed, youth, justice-involved, English as a second language, women, and children, is required.
Self-starter, ability to work with minimal supervision; excellent communication, multi-tasking, community relations, networking, and public speaking skills.
Demonstrated ability to work with families, women, and children without discrimination towards people of diverse cultures, races/ethnicities, socio-economic positions, ages, religions, genders, physical/mental challenges/disabilities, and sexual orientations.
Understand, explain, and apply complex local, state, and federal regulations, policies, and procedures.
Able to travel to meet with service providers and participants; ability to work independently as well as part of a team; must have a flexible schedule on evenings and weekends.
Proficient computer skills in MS Word, Excel, PowerPoint, database management, and Internet Explorer.

Responsibilities

Conduct comprehensive health and social needs assessments and develop individualized care plans.
Coordinate services across medical, behavioral health, housing, social services, and other systems of care.
Provide direct case management support using trauma-informed and strengths-based approaches.
Implement outreach and engagement strategies targeting individuals experiencing homelessness or at high risk.
Support clients through housing assessments, navigation, and placement to achieve housing stability.
Identify and maintain landlord relationships to expand housing availability for program participants.
Maintain timely and compliant documentation per CalAIM, HIPAA, MCP guidelines, and agency standards.
Track and report outcomes, analyze service metrics, and contribute to performance improvement.
Participate in CalAIM Learning Collaborative, stakeholder convenings, and interagency partnerships.
Provide on-site leadership during emergencies, including de-escalation and crisis resolution.
Model adherence to evidence-based practices and agency protocols.
