About The Position

As a Lead Care Manager I, you will: Provide mobile outreach and engagement Screen and assess for program eligibility Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals Connect clients and their families to a medical clinic and providers, as well as other clinic and community-based services Arrange transportation and accompany clients to office visits as needed Serve as an advocate for the client and family Use motivational interviewing, trauma-informed care, and harm-reduction practices Create and maintain all required documents in the electronic and paper record systems Healthy Connect San Diego is a great place to grow, come join our awesome team! Healthy Connect Youth Services is a program of Healthy Connect San Diego. We provide care coordination to people with complex physical and behavioral health needs. Requirements

  • High School/GED diploma with 2 years of experience working in social services or related setting required.
  • A valid California driver's license in good standing, personal vehicle, proof of insurance and eligibility to be a CRF Approved Driver are required to fulfill the essential functions of this position
  • Ability to pass criminal/background clearance
  • Proficient computer skills, including MS Office

Nice To Haves

  • Bachelor's degree in Public Health, Social Work, Psychology, Human Services or related field with 1 year of experience working in social services or related setting strongly preferred.
  • At least one year of experience working with youth strongly preferred
  • Experience providing case management services preferred
  • Experience working with Seriously Mentally Ill (SMI) population is preferred
