Position Requirements:

Requirements

• Minimum Education Required: Bachelor's Degree. Preferred: Master's Degree
• Minimum Experience Required: 6-10 years of previous job-related experience with 3-5 years in sub-acute, acute inpatient rehabilitation or similar setting.
• Experience Details: Ambulatory or Case Management Experience: 1-2 years
• Licensure/Certifications: Required: Current Registered Nurse License State of Illinois.
• Desired: national certification in case management.
