This is a remote position. LCSW Conduct virtual initial assessments and ongoing care for seniors with serious mental illness (SMI) and substance use disorders (SUD), in collaboration with psychiatrists and other healthcare professionals. Develop comprehensive treatment plans, which may include coordinating psychotherapy or other treatment modalities, and assessing the need for referrals to specialized care. Collaborate effectively as part of a multidisciplinary team to address the holistic needs of patients, including their physical health, behavioral health, and social well-being. Provide patient care and education tailored to the individual’s age, abilities, educational background, and cultural context, often involving family members in the care process. Coordinate care with primary care physicians (PCPs) and other medical professionals to ensure a cohesive approach to patient health. Offer timely psychiatric urgent care and be responsive to patients’ immediate needs. Document all clinical work comprehensively and promptly in the electronic health record (EHR). Ability to effectively engage and build rapport with challenging patients, using a solution-focused and time-limited treatment model to achieve tangible, positive outcomes. Eagerness to engage with a high-needs patient population while maintaining quality care and positive outcomes.
