About The Position
Under the supervision of the Behavioral Health Supervisor and or his/her designee, the Licensed Clinical Social Worker will provide concrete services and therapeutic interventions for patients and their family members. Treatment services will include psychosocial screenings, assessments, and case management functions. Provide direct service to patients and families. The social worker will participate in multidisciplinary case conferences, attend provider meetings, and perform behavioral health integration activities with the goal of primary care - mental health integration.
