The LCSW/LMFT provides comprehensive psychosocial and clinical assessments to meet the care needs of the patients that Kaweah Health serves. This role involves providing education, referral, and support in a psychosocial aspect in the situations of individuals, families, or groups, helping to improve social or health services. The position requires a Licensed Clinical Social Worker (LCSW) or Licensed Marriage Family Therapist (LMFT) and mandates Kaweah Health approved crisis intervention training within 60 days of hire or transfer. The LCSW/LMFT is responsible for gathering information about the patient's social, psychological, cultural, environmental, and financial situation for a psychosocial assessment and treatment plan. They will counsel individuals and their families regarding psychosocial needs and problems related to their medical condition, provide discharge planning, and coordinate necessary follow-up services. The role also includes intervening in crisis situations, providing consultation to physicians and hospital staff, and making rounds to respond to referrals from physicians and staff. The LCSW/LMFT will also be involved in community outreach programs, providing education on psychosocial aspects of illness, and ensuring that all patient information is documented accurately according to procedure.
