Licensed Clinical Social Worker provides clinical social work services to patients and their families. Provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems. Coaches patients on how to manage ongoing health conditions and provides therapy to help patients make positive behavior changes.

Requirements

  • Requires a master’s degree in social work.
  • Additionally, requires an Illinois state license to practice Clinical Social Work.
  • Three or more years of practice as a Licensed Clinical Social Worker
  • Certification and or licensing in the position's specialty is the main requirement.

Responsibilities

  • Working with SNF’s, Home Health Agencies, Department of Child and Family Services, Department of Human Services, Mental Health Center, Showbus, Volunteer Services, Adoption Agencies, DME providers, VA Centers, Other hospitals
  • Demonstrates knowledge and application of the JCAHO performance improvement standards in both the hospital and resident home.
  • Compiles PI data and prepares quarterly PI reports which are submitted to the Director.
  • Assist the Director to prepare an annual PI summary and to develop the PI plan for the coming year.
  • Refers inquiries from patients and families concerning quality issues to the appropriate Department Director.
  • Participaties in the Performance Improvement activities and serves as a member of a Task force or committee, when appropriate, in order to improve processes and quality of Care.
  • Attends daily unit huddle to report discharges and concerns
  • Maintains compliance with current regulatory requirements of State and JCAHO standards regarding a Social Service Worker.
  • Answers phones and greets visitors
  • Focuses on the patients social, financial and discharge planning needs as follows: responds to referrals, financial assessment of patients to assist in their healthcare discharge needs, document interactions in the EMR -Social Service Notes, assesses patients on their health condition and understanding of their discharge plan, obtaining all critical information to assist the patient in their discharge plan. Discussion with other disciplinary teams involved in the patients care to determine discharge needs.
  • Assists patients and families with Healthcare POA’s.
