About The Position
McLean Home Care and Hospice is looking for a 24 hour - Licensed Clinical Social Worker (LCSW) to assist with assessing our patients and family's needs - psychosocial, environmental, and financial. The LCSW will formulate, implement, and evaluate a plan of care in collaboration with the patient, family, and the caregiving team. Proper documentation per regulatory standards is required. The role also involves monitoring, observing, and evaluating changes and progress in the patient's condition and environment, and collaborating with the caregiving team.
