The LAHSA TLS SCI Generalist is a field-based role focused on providing comprehensive support to homeless individuals participating in Project Room Key. This position involves outreach, case management, housing application assistance, and connecting clients to various supportive services. The goal is to facilitate housing stability and improve the overall well-being of clients through advocacy and resource coordination.

  • Provide outreach to referred homeless individuals to screen for COVID LAHSA TLS Program.
  • Establish and maintain knowledge of community services and communicate appropriate referrals to members.
  • Conduct regular case management meetings with program participants to encourage self-determination.
  • Advocate for clients' needs and connect them with supportive services.
  • Search for housing options aligned with clients' capacities and choices.
  • Conduct HQS inspections to ensure housing meets HUD quality standards.
  • Coordinate lease signings and assist members with the moving process.
  • Document every client contact and maintain accurate client files in HMIS.
  • Ensure timely entry of data in HMIS and track client outcomes.
  • Bachelor's degree in Social Work or related field.
  • Minimum of 1 year experience working with homeless individuals or those with mental illness.
  • Bilingual in Spanish is required.
  • Strong computer skills, including proficiency in Microsoft 365 Office.
  • Experience in crisis intervention and case management.
  • Knowledge of mainstream benefits and services.
  • Familiarity with Housing First and Harm Reduction models.
  • Experience with Motivational Interviewing and Trauma Informed Care.
  • Competitive hourly wage of $25.48 - $26.44.
