About The Position

The Kentucky Veterans Center is seeking a dedicated Social Worker Specialist to provide professional social work and counseling services to veterans and facility staff. Requirements

• Master's degree in social work.
• Two years of professional social work experience.
• Must be licensed in Kentucky as a social worker by the Kentucky Board of Social Work.
• Strong attention to detail.

• Strong organizational skills and ability to multi-task.
• Proficient computer skills.
• Exceptional problem-solving skills.

Responsibilities

• Provides professional social work and counseling services to the veterans and facility staff.
• Coordinates resident interaction with family and community support groups.
• Monitors the residents' social and emotional state, adjustment to the nursing home environment and treatment progress.
• Documents psychosocial histories, treatment plans, discharge plans and other pertinent data.
• Performs other duties as assigned.
