At FBH we have a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement and more. Our evidence-based, culturally appropriate, trauma informed behavioral health services address the diverse needs of those we serve. Kinship Navigators will provide unpaid caregivers and Kinship families assistance in gaining access to services that support them in their caregiving role, including information about available public and private long-term care support services, (i.e., supportive counseling, caregiver training, support groups, medication education), and supplemental services as needed and appropriate. Facilitate completion of the Caregiver Survey and conduct TCARE assessments to determine service eligibility. Facilitate caregiver support groups and educational classes and workshops. As appropriate, link caregivers to mental health services and other services deemed relevant and necessary. Employ special case finding, education and outreach efforts that include distributing kinship family information to the community, networking with and educating selected community agencies regarding resources and services available to Kinship families. As a Kinship Navigator, this staff’s role is providing case management for families raising relative children (Kinship Families) when the primary parents are not able to provide appropriate care. This includes, but not limited to providing information and referral to community resources, locating long-term services and supports, coordinating with schools and other community resources, and when eligible, providing financial assistance. Provide case management for Family Caregiver Support Program clients by providing services that includes resources and information, respite, and supplemental services.
