At FBH we have a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement and more. Our evidence-based, culturally appropriate, trauma informed behavioral health services address the diverse needs of those we serve. Kinship Navigators will provide unpaid caregivers and Kinship families assistance in gaining access to services that support them in their caregiving role, including information about available public and private long-term care support services, (i.e., supportive counseling, caregiver training, support groups, medication education), and supplemental services as needed and appropriate. Facilitate completion of the Caregiver Survey and conduct TCARE assessments to determine service eligibility. Facilitate caregiver support groups and educational classes and workshops. As appropriate, link caregivers to mental health services and other services deemed relevant and necessary. Employ special case finding, education and outreach efforts that include distributing kinship family information to the community, networking with and educating selected community agencies regarding resources and services available to Kinship families. As a Kinship Navigator, this staff’s role is providing case management for families raising relative children (Kinship Families) when the primary parents are not able to provide appropriate care. This includes, but not limited to providing information and referral to community resources, locating long-term services and supports, coordinating with schools and other community resources, and when eligible, providing financial assistance. Provide case management for Family Caregiver Support Program clients by providing services that includes resources and information, respite, and supplemental services.

Requirements

  • A Master’s degree in behavioral or health sciences and one year of paid on-the-job social service experiences; OR A Bachelor’s degree in behavioral or health sciences and two years of paid on-the-job social service experience; OR A Master’s in any other field and three years of paid on-the-job social service experience; OR A Bachelor’s degree in any other field and four years of paid on-the-job social service experience.
  • Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire.
  • Must have an unrestricted and valid driver’s license, active car insurance and must be willing to use their personal vehicle in the course of work.
  • Knowledge of elder care and disabled adult caregiving issues.
  • Knowledge of kinship care, children and families as well as elder care and disabled adult caregiving issues.
  • Ability to perform face-to-face assessments and screenings.
  • Proficiency with Microsoft Office suite and experience with database systems.
  • Experience with leading large groups and public speaking.

Responsibilities

  • Facilitate completion of the Caregiver Survey, TCARE assessment, consultation, and Care Plans within required timeframe.
  • Provide information to the public about FCSP and KCSP services, including brochures, presentations and participate in the planning of outreach events.
  • Provide unpaid caregivers and Kinship families with information, referrals to community resources, and assistance in gaining access to an array of appropriate services.
  • Authorize Kinship Caregiver Support Program (KCSP) funds to eligible clients.
  • Utilize a variety of screening tools and assessments to develop care plans.
  • Develop, organize and facilitate caregiver support groups.
  • Organize and facilitate educational classes and workshops.
  • Provide unpaid caregivers and kinship families access to information that assists them in making decisions and solving problems related to their caregiver roles.
  • Complete documentation regarding units of service for all Core service areas, unmet needs and supplemental requests.
  • Provide advocacy as appropriate.
  • Provide timely referrals to appropriate caregiver-related services.
  • Develop working relationships with key personnel within the criminal justice system and Spokane County schools.
  • Ensure an in-depth knowledge of community resources/services related to legal issues.
  • Ensure required documentation including demographics and program metrics are entered into GetCare.
  • Complete Mini Assessment in GetCare for kinship families.
  • Represent FBH in a positive, compassionate and professional manner.
  • Represent FBH by participating in public forums, speaking engagements and informational meetings.
  • Participate in Peer Review, supervision, intake and diagnostic staffing, in-center educational meetings as directed.
  • Support and promote positive teamwork.
  • Comply with WAC requirements and FBH policies and procedures.
  • Maintain and enhance customer service to ensure that external and internal FBH customers have a positive experience.
  • Perform FBH and ALTCEW charting requirements, correspondence, and other paperwork in a complete and timely manner.
  • Requires travel to see clients in the community and travel between work sites.
  • Other duties as assigned.

  • Benefits described above are based on employees who hold .5 FTE positions or greater and/or some benefits vary based on FTE. PRN positions are not benefit eligible or eligible for a sign on bonus.
