The clinical transplant coordinator is a professional registered nurse and a designated member of the transplant team who is responsible for coordination of services necessary to determine suitability of potential recipient’s candidacy for transplantation as well as potential living donors, and maintenance of optimal health status during potential recipient’s waiting period on the UNOS wait list. The clinical transplant coordinator works with patients and families beginning with the evaluation phase for transplantation and continuing through and following transplantation in a compassionate and tactful manner in order to facilitate access to and provide continuity of care. Responsible for management of post transplant care for the lifetime of the patient. The coordinator also works with other members of the transplant team, including surgeons, physicians, mid-level practitioners, nurses, social workers, financial coordinators and administrative personnel. Maintains an active role in educating the hospital and public about current information regarding organ donation and transplantation.
