About The Position

The Patient Education Programs Educator will be instrumental in helping to drive volume of CKD participants educated to the Kidney Smart programby conducting direct education virtually, support with core operations for the program and provide increased capacity for our field teammates across the country to increase the number of participants receiving lifechanging CKD education through Kidney Smart.

Responsibilities

• Provide telephonic callsupportto CKD participantsin focused areas acrossthecountry including education,scheduling, rescheduling, and targeted Spanishsupport.
• Review, educate and answer questions on all modality options for dialysis including transplant, hemodialysis, PD and HHD
• Provide local coveragesupportwith opportunities and data entry as needed. . Completedata entry&tasking into data managementsystem.
• Other duties as assigned.
