Become a part of our caring community The Nephrology Nurse is a specialized Registered Nurse who supports patients with chronic kidney disease (CKD), dialysis, and transplant care. This role combines clinical expertise with care coordination, guiding patients through complex healthcare systems and ensuring timely, patient-centered interventions. The Nephrology Nurse collaborates with multidisciplinary teams, to improve outcomes and enhance quality of life for individuals with kidney-related conditions. Requirements

  • Active Registered Nurse (RN) license in the state of practice.
  • Minimum 5 years of experience in nephrology, dialysis, or transplant nursing.
  • Strong clinical knowledge of kidney disease management.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficiency in electronic medical records (EMR) and healthcare IT systems.
  • Ability to work independently and collaboratively.

Nice To Haves

  • Bachelor of Science in Nursing (BSN).
  • Certification in Nephrology Nursing (CNN/CDN) or Case Management (CCM).
  • Experience in value-based care or population health programs.
  • Familiarity with community resources and patient education tools.
  • Bilingual proficiency is a plus

Responsibilities

  • Conduct comprehensive assessments of patients' kidney health and care needs.
  • Develop and implement individualized care plans in collaboration with nephrologists and care teams.
  • Educate patients and families on CKD, dialysis, transplant options, and self-management strategies.
  • Coordinate referrals, appointments, follow-ups, and transportation.
  • Assist patients in navigating healthcare systems, including insurance and documentation.
  • Monitor patient progress and respond to changes in condition.
  • Maintain accurate documentation in EMR systems, ensuring compliance with HIPAA and clinic policies.
  • Participate in quality improvement initiatives and help define workflows and best practices for kidney care navigation.
