About The Position

The Kidney Care Advocate position is designed to enhance the patient experience for individuals living with kidney disease. Under the guidance of a Kidney Care Advocate Manager, this role involves collaboration with care teams to provide education on treatment options, facilitate access to care, and support patients through their journey in kidney care services. The advocate will also assist in presenting educational materials and ensuring comprehensive support for patients and their caregivers.

Requirements

  • High School Diploma or equivalent; college degree preferred.
  • Minimum of one year experience in patient interaction, teaching, or sales; dialysis experience preferred.
  • Demonstrated customer service skills and ability to work autonomously.
  • Excellent verbal, written, and presentation skills.
  • Proficient in Microsoft Outlook, Excel, Word, PowerPoint, and smart devices.
  • Ability to multi-task and manage cases in a Salesforce database or CRM system (preferred).
  • Results-oriented with a focus on exceeding established goals.
  • High level of emotional intelligence and ability to relate to diverse individuals.
  • Proven ability to work as a team player.

Nice To Haves

  • CCHT and/or LPN certification preferred.

Responsibilities

  • Collaborate with the Regional Home Therapy team to provide administrative support and ensure comprehensive education on modality options for patients and caregivers.
  • Provide information regarding renal replacement therapy options to patients reaching end stage renal disease (ESRD).
  • Assist patient care teams in navigating patients through the initiation of dialysis and transitions to home therapy.
  • Maintain communication with physicians, physician practices, and interdisciplinary team members.
  • Establish processes for patient referrals and evaluate the effectiveness of these processes.
  • Respond promptly to all referrals and coordinate timely admissions for patients into Home Therapy and In-Center programs.
  • Identify training delays in home therapy and work to resolve barriers to expedite training.
  • Promote awareness of Fresenius Kidney Care Home Therapies and identify opportunities for increasing patient education referrals.
  • Support the implementation of the home therapy strategic plan and understand local kidney disease trends.
  • Maintain knowledge of FMCNA Home Therapies products and promote them through community outreach programs.
  • Support educational programs for physicians and Fresenius staff as needed.
  • Ensure accurate documentation of patient interactions and status in the SAP database.
  • Escalate issues to the supervisor for resolution as necessary.
