About The Position

The Memory Care Resident Care Coordinator is responsible for coordinating, monitoring, and supporting the delivery of high-quality, person-centered care to residents living with Alzheimer’s disease and other forms of dementia. This role serves as a key liaison among residents, families, caregivers, clinical staff, and leadership to ensure care plans are implemented effectively, regulatory standards are met, and residents maintain the highest possible quality of life.

Requirements

  • High school diploma or GED required; Associate’s or Bachelor’s degree in healthcare, social work, or related field preferred
  • Experience in memory care, assisted living, long-term care, or healthcare coordination preferred
  • Dementia care training or certification strongly preferred
  • Strong understanding of Alzheimer’s disease and dementia-related behaviors
  • Excellent communication, interpersonal, and organizational skills
  • Ability to work compassionately with residents, families, and staff
  • Proficiency with electronic health records and documentation systems
  • Ability to multitask and prioritize in a fast-paced environment
  • Ability to stand, walk, bend, and assist residents as needed
  • Ability to lift or assist with lifting up to [X] pounds with or without accommodation

Responsibilities

  • Resident Care Coordination Coordinate and oversee individualized care plans based on resident assessments, physician orders, and family input
  • Monitor residents’ physical, emotional, and cognitive status and report changes promptly
  • Ensure care plans are updated regularly and communicated clearly to caregiving staff
  • Support resident transitions, including move-ins, room changes, and care level changes
  • Staff Support & Collaboration Act as a resource for caregivers regarding dementia care best practices
  • Provide guidance and coaching to care staff to ensure consistent, compassionate care
  • Collaborate with nursing, life enrichment, dining, and housekeeping teams
  • Assist with staff training related to memory care protocols and resident needs
  • Family & Resident Communication Serve as a primary point of contact for families regarding care-related concerns
  • Participate in care conferences and family meetings
  • Educate families on dementia progression, care approaches, and available resources
  • Address concerns or issues with professionalism and empathy
  • Compliance & Documentation Ensure compliance with state regulations, licensing standards, and company policies
  • Maintain accurate resident records, assessments, and care documentation
  • Assist with audits, inspections, and quality assurance initiatives
  • Quality of Life & Engagement Support meaningful engagement and daily routines tailored to resident abilities
  • Collaborate with Life Enrichment to promote purposeful activities
  • Advocate for dignity, safety, and resident-centered decision-making
