About The Position

The Job Coach/Transition Specialist at The Arc Erie County New York plays a crucial role in supporting individuals with developmental disabilities in achieving their vocational potential. This position involves working directly with clients to develop essential work and social skills, facilitating their integration into the workforce, and serving as a mentor and advocate in various community settings.

Requirements

• Associate degree and 1 year of full-time direct service experience, or
• High School diploma or equivalent and 2 years of full-time direct service experience.
• Valid NYS Driver's License and approved driving record.

Nice To Haves

• Experience working with developmentally disabled individuals is preferred.

Responsibilities

• Instruct and assist individuals in developing social skills, work skills, and work habits in a community-based setting.
• Work one-on-one with individuals to complete tasks and provide a positive work model.
• Orient individuals to new work settings, including work routines, production demands, and use of equipment.
• Monitor the quantity and quality of work produced by individuals at community business sites.
• Transport/escort individuals to and from community work sites and provide mobility training as required.
• Work with individuals on defined vocational goals and assess progress monthly, documenting in progress notes.
• Assist individuals in integrating into the workforce through natural supports and social participation with co-workers.
• Participate in case conferences, in-service training, and consultations with the individual's Program Manager.
• Act as a liaison between individuals, employers, and The Arc Erie County New York.
• Advocate for safe and healthy work conditions and report issues to the Supervisor.
• Make job carving recommendations to adapt job duties to the abilities of individuals.
• Complete daily notes and monthly summaries reflecting services provided and individual progress.
