About The Position

The Jail Case Manager – Specialty Dockets is responsible for coordinating case management services for incarcerated individuals participating in specialty court dockets. This position works collaboratively with the courts, jail staff, behavioral health providers, and community partners to support compliance with court requirements, address criminogenic needs, and promote successful outcomes during incarceration and reentry.

Requirements

• AA/BA degree in social work, Criminal Justice, Psychology, Human Services, or a related field.
• Experience in case management, corrections, behavioral health, or social services.
• Valid driver's license and insurance.
• As a healthcare provider, Southeast Healthcare is a non-smoking employer.

Responsibilities

• Manage assigned caseload of incarcerated individuals enrolled in specialty dockets (e.g., Mental Health Court, Drug Court, or Veterans Court).
• Conduct screenings and assessments to identify treatment, programming, and reentry needs.
• Develop and monitor individualized case plans aligned with court orders and docket requirements.
• Coordinate services with internal jail programs and external community providers, including behavioral health, substance use treatment, housing, and benefits.
• Serve as a liaison between the jail, courts, probation/parole, attorneys, and treatment providers.
• Maintain accurate and timely documentation in case management and jail management systems.
