About The Position

The IRIS Consultant (IC) position at TMG involves partnering with individuals enrolled in the Wisconsin IRIS Program to help them navigate long-term care options. This remote role requires building relationships with participants, assisting them in developing customized plans to achieve their goals related to various aspects of life, and connecting them to community resources. The IC plays a crucial role in supporting individuals with disabilities and older adults, ensuring they receive the necessary guidance and support to live fulfilling lives.

Requirements

  • Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field, or four years of relevant experience.
  • 1+ year of direct experience in delivering social services to individuals with disabilities and older adults.
  • Ability to work independently with minimal supervision and be self-motivated.
  • Knowledge of Long-Term Care programs and self-determination principles.
  • Excellent problem-solving, critical thinking, and basic math skills.
  • Strong time management and prioritization skills to handle multiple projects.
  • Ability to develop and maintain professional relationships and manage situations effectively.
  • Comfortable working with diverse populations and adjusting communication styles as needed.
  • Resourceful with knowledge of community resources and detail-oriented.
  • Proficient in using technology including Outlook, Skype, Teams, PowerPoint, Excel, Word, and online databases.

Responsibilities

  • Meet in person with IRIS participants a minimum of four times per year, including one required annual home visit.
  • Provide program orientation to new participants, explaining their rights and responsibilities.
  • Explore participants' lives to identify goals, relationships, and community connections.
  • Assist participants in identifying personal outcomes and ensure they are met within their IRIS budget.
  • Document all orientation and planning activities in the IRIS data system within 48 business hours of visits.
  • Research and share community resources that align with individual outcomes for participants.
  • Document progress and changes in the plan and data system as requested by participants.
  • Educate participants on interpreting their monthly budget reports and assist with provider billing and payments.
  • Develop trusting relationships with participants and communicate program changes effectively.
  • Maintain confidentiality and HIPAA compliance.
  • Collaborate with other IRIS Consultant Agency staff for successful plan implementation.
  • Attend monthly team meetings and participate in weekly check-ins with supervisors.
