About The Position

Glendale Memorial Hospital and Health Center is seeking a dedicated and experienced Acute Rehabilitation Unit (ARU) Assessor to join our dynamic team. This is a pivotal role that directly impacts patient care and ensures our ARU operates at the highest standards of quality and compliance. Assesses patients for admission to Acute Rehab Unit (ARU). Educates healthcare workers regarding medical necessity documentation and functional improvement measures. Submits accurate reports per regulation within defined timeframes. Demonstrates adherence to the standards of conduct requirements. Demonstrates the values of the organization: Collaboration, Dignity, Excellence, Justice and Stewardship. Has a positive attitude and sense of ownership in the organization. Promotes communication that supports agreeable solutions. Is always willing to assist. Evaluates patients prior to admission for appropriateness of admission to Acute Rehab Unit (ARU). Visits patients on Acute to describe the program and evaluate appropriateness of patient for the program (medical necessity, willing & able to participate). Recognizes any cultural needs of patient and family. Talks with the family to determine any special needs and to evaluate/confirm the discharge plan. Asks for guidance if unsure. Facilitates admissions of appropriate patients to the ARU from GMHHC acute. Talks with Patient Flow Coordinator to see if there are patients for Rehab. Facilitates admissions of appropriate patients to the ARU from external sources. Communicates verbally, by phone, in writing and at times in person with external sources. Facilitates documentation of medical necessity and comorbidities. Educates all disciplines initially on hire and on an ongoing basis regarding ARU requirements and FIM scoring. Facilitates documentation of functional improvement measures (FIM). Reviews medical records of ARU patients daily for first three days and on the day before discharge for appropriate documentation to support FIM Score. Gives feedback to care provider. Escalates issues of non-compliance to supervisor. Educates all disciplines regarding the FIM, including: a. how to assess patient to determine FIM Score b. how to document patient assessment/interventions to support FIM c. how FIM affects IRF PAI and reimbursement.

Requirements

  • Current and active CA,Registered Nurse Licensure
  • Minimal education as required for licensure
  • AHA Basic Life Support

Nice To Haves

  • Preferred 1-3 years
  • One year experience with Functional Improvement Measure (FIM) assessment, upon hire
