This role coordinates surveillance, imaging, reviews, and patient communication for high-risk patients, ensuring they follow evidence-based guidelines and receive timely follow-ups. The position works with Gastroenterology, Surgery, Oncology, Radiology, APP leadership, and executives to maintain standardized workflows, track patients long-term, and continuously improve the ISE Program. POSITION SUMMARY This position provides programmatic, operational, and quality coordination support for patients across the Integrated Specialty Enterprise (ISE). This role serves as the central point of coordination for surveillance, imaging, multidisciplinary review, and patient communication to ensure adherence to evidence-based guidelines and timely follow-up for a high-risk patient population. This position collaborates closely with Gastroenterology, Surgery, Oncology, Radiology, APP leadership, and executive stakeholders to support standardized workflows, longitudinal patient tracking, and continuous quality improvement within the ISE Program.
