This role coordinates surveillance, imaging, reviews, and patient communication for high-risk patients, ensuring they follow evidence-based guidelines and receive timely follow-ups. The position works with Gastroenterology, Surgery, Oncology, Radiology, APP leadership, and executives to maintain standardized workflows, track patients long-term, and continuously improve the ISE Program. Location: This position will float from your remote office to the Mesa and Gilbert clinic Schedule: Monday - Friday 8:00am - 4:30pm At Banner Medical Group, you'll have the opportunity to perform a critical role in the community where you practice. POSITION SUMMARY This position provides programmatic, operational, and quality coordination support for patients across the Integrated Specialty Enterprise (ISE). This role serves as the central point of coordination for surveillance, imaging, multidisciplinary review, and patient communication to ensure adherence to evidence-based guidelines and timely follow-up for a high-risk patient population. This position collaborates closely with Gastroenterology, Surgery, Oncology, Radiology, APP leadership, and executive stakeholders to support standardized workflows, longitudinal patient tracking, and continuous quality improvement within the ISE Program.

Requirements

  • Associate degree in Science, Healthcare, Education or related or equivalent experience.
  • Requires two years of program coordination or healthcare administrative experience.
  • Proficiency in Microsoft Office and EHR systems.
  • Strong communication skills.

Nice To Haves

  • Experience in GI, oncology or complex care programs.
  • Familiarity with multidisciplinary care coordination and quality improvement initiatives.
  • Additional related education and/or experience preferred.

Responsibilities

  • Serves as the primary coordinator and point of contact for patients across the ISE.
  • Coordinates and tracks surveillance workflows in alignment with national guidelines.
  • Maintains and manages a centralized ISE patient registry/database.
  • Identifies patients who have fallen out of recommended surveillance pathways.
  • Coordinates and supports multidisciplinary case review meetings.
  • Collaborates with physicians, APPs and clinical leaders to implement care plans.
  • Monitors program volumes, compliance metrics and outcomes.
  • Identifies quality, safety and operational improvement opportunities.
  • Provides patient education and resource support.
  • Performs administrative and program support functions as needed.
