About The Position

The Intersection to Care Peer Liaison is to provide women leaving incarceration with the resources and skills to reintegrate into the community and obtain meaningful employment. ICT Peer Liaison will be responsible for evaluating and meeting with women, pre-release, to coordinate and provide direct resources, referral to community assistance, and reintegration services for women being housed in the Shawnee County Adult Detention Center (SCADC), thereby increasing their opportunity to become productive members of the community, and thereby reducing recidivism. ICT Peer Liaison’s shall operate in accordance with the policies set forth by the Board of Directors and as implemented by his/her supervisors and they shall abide by all applicable laws and regulations. This position will be located inside the Shawnee County Jail.

Requirements

High school diploma or General Educational Development certificate.
Kansas Certified Peer Mentor (KCPM) or Kansas Certified Person-Centered Case Manager (PCCM) preferred or eligible to become one.
Experience in supervising and interacting with persons addicted to drugs or the prison population would be desirable.
Must be proficient in computer skills and/or experience or skills with an Electronic Health Record.
Good reading, writing and communication skills.
Ability to organize and prioritize and follow through with multiple tasks related to client charting.
Must be able to pass background checks authorized by the agency.
Employees who have been diagnosed with any type of substance use disorder or behavioral addiction/compulsion must have sustained a minimum of two consecutive years of abstinence from all intoxicating substances and symptoms of their chemical and/or behavioral addiction; and must be in full sustained remission.
Job functions in this position may involve some walking, climbing stairs, bending, lifting (or assisting) up to 40/50 pounds.
If person is a former client of this agency then they need to have been discharged from here for at least 24 months.

Responsibilities

Enroll participants in the ITC program and offer relevant services to eligible and interested participants of the Intersections to Care (ITC) Program.
Collaborate and work directly with SCADC personnel, parole, and probation to evaluate needs of each individual and create individualized ITC Plans.
Coordinate care for clients leaving incarceration. This includes collaborating with local organizations to develop and maintain access to services for women leaving incarceration, including housing, food and clothing, mental/physical health care, substance use treatment, employment, transportation, training and education or other services as needed.
Schedule appointments, services and transportation as needed before a participant leaves incarceration.
During first 4 weeks post incarceration, follow-up with participants or their mentor weekly as a minimum. Follow-up monthly once client has reached stabilization. Record each contact in the tracking spreadsheet.
Maintain client and program confidentiality as required by federal laws.
Facilitate group meetings three (3) times per week for women incarcerated at SCADC. Group meeting content to be approved by SCADC and ITC program managers.
Coordinate the ITC Mentor program. Facilitate enrolling interested participants in the Mentor program, provide information about potential Mentees to Mentor group, assist in recruiting Mentors, coordinate and attend bi-weekly Mentor meetings. Facilitate development of strong ITC Mentor program.
Participate in any staff meetings and community meetings as required by your supervisor.
Ensure proper documentation of all services.
Provide reports on program metrics as required.
Develop intervention plans to prevent or reduce recidivism.
Perform other job-related functions as directed by your immediate supervisor
