Shawnee Health is seeking an Interpreter/Outreach Worker for our Shawnee Health Murphysboro office. Under the supervision of The Farmworker Program Coordinator and with guidance from the Health Center Manager, the Outreach Worker/Interpreter will provide interpretation, outreach and social service assistance to Migrant and seasonal farmworker patients of the health center through case management and coordination with the rest of the staff. This position must be capable of linking the patient/client to other appropriate services in the Shawnee Health Service network or in the community, and to provide interpretation and health education to patients when necessary. This position is responsible for ensuring that case management and social services are being provided effectively for all Migrant and seasonal Farmworker patients who need those services.

Requirements

  • Must have a valid Illinois driver's license and auto liability insurance on personal vehicles.
  • May be required to work a flexible schedule, including some evenings and/or weekends, as well as early morning hours.
  • Reliable personal transportation is mandatory
  • High School Diploma or equivalent
  • Advanced level of oral fluency in English and Spanish
  • Good reading and writing skills in Spanish
  • Excellent English Communication skills both written and Oral
  • Excellent interpersonal and organizational skills
  • Ability to maintain patient and employee confidentiality
  • Competent in the use of computers, including Microsoft Word
  • Capable of independent problem solving with moderate supervision
  • Employee must pass all pre-employment requirements as delineated below. Employee's failure to pass all pre-employment requirements as determined by Shawnee Health shall result in employee's immediate termination, or, if not yet hired, withdrawal of job offer.

Responsibilities

  • Represents Shawnee Health in the most positive manner with prospective, former and current patients, clients, their families and the community we serve.
  • Upholds the Mission Statement-The fundamental mission of Shawnee Health is to improve the health and welfare of southern Illinois and southwest Indiana residents through the promotion, development and administration of quality, preventive primary health and social services, while efficiently utilizing limited resources. This will include assessing and serving the needs of the underserved/vulnerable populations and designing programs and services which are culturally and linguistically appropriate.
  • Handles confidential information with tact and discretion.
  • Maintains appropriate levels of bilingual medical terminology proficiency through consistent review, study and assessments, as well as participation in trainings, workshops or any other training opportunities assigned by the supervisor to provide quality medical interpretation to our patients
  • Organizes schedule to ensure that all appointments requiring a language interpreter are covered with either an in-person or a phone interpreter accordingly.
  • Maintains updated and relevant bilingual health education materials for topics appropriate to assigned clinic. Delivers one-on-one health education to all patients for issues identified as much as schedule permits.
  • Provides outreach services to the underserved members of Shawnee Health's target community. Provides information to the general population and other community agencies regarding SHS and our health related topics. Represents SHS and assigned clinic or program at health fairs and other community events.
  • Serves as a patient advocate in seeking resources or services for which the patient may be eligible. In accordance with confidentiality of patient information policies, communicates with external resources or agencies for the purpose of case coordination.
  • Covers weekly shifts for mandatory On-Call Program to provide telephone interpretation for Spanish speaking patients calling to speak with the on-call provider. The designated on-call person will carry a phone and accept calls from all patients calling after hours. (See Mandatory On-call Program Policy).
  • Ensures arrangements for transportation are provided to Farmworker patients who request the service as resources and schedule permit ensuring safe and quality transportation (see Farmworker Patient Transportation Program Policy).
  • Assists health center patients in identifying psychosocial, environmental, and economic issues impacting on patient's ability to maintain a functional health status. Assists health center patients and family/significant other in prioritizing needs and, as feasible, establishing a care plan with measurable goals and timeframes to meet these needs. Encourages family/significant other involvement in resolution of patient's needs, when appropriate.
  • Oversee the utilization of, and referral to, programs available to assist patients with financial needs such as SFAP, and others. Assists with or arranges for assistance with applications for state or federal benefits (Allkids, Medicare, food stamps, LIHEAP, DOL, Breast and Cervical Cancer program, etc.)
  • Refers patients to other providers within Shawnee Health (e.g., Shawnee Alliance for Seniors, Healthy Families, etc.) as well as to resources within the community (e.g. Public Health, Mental Health, DORS, Women's Center, DHS, DCFS, Food Banks, Senior Citizen Centers, school systems, law enforcement agencies, Migrant Legal Assistance, SuCasa Migrant Head start, etc.) as in accordance with the care plan.
  • Performs regular checks of program vehicles responsibly assigned by program supervisor
  • Performs other duties as assigned or requested.
  • Employee must maintain regular, punctual attendance consistent with Shawnee Health policies and procedures. Employee must report to work and be on time for each scheduled shift unless scheduled absent or on approved leave in accordance with federal and/or state law. Regular employee attendance is essential to the efficient operation of Shawnee Health Service and is an essential component of employee's job duties.
  • Employee will promote a culture of continuous improvement by actively participating in team-based improvement events that will support SHS process improvement goals and initiatives.
  • Employee will maintain compliance with Shawnee Health policies, procedures, and on-going education requirements. Employee will maintain compliance with program specific standards and regulations.
  • Employee will comply with all standard operation procedures of Shawnee Health regarding employee health and safety, including but not limited to OSHA regulations (Bloodborne Pathogen Standard and Hazardous Communication Rules), Environmental Protection Act (EPA) regulations governing waste disposal, and completion of unusual occurrence reports.
