The Internist is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care. This role involves the direct provision of primary care medical services, meeting all established productivity expectations, and includes responsibilities such as examining patients, reviewing laboratory and imaging results, and documenting visits accurately. The Internist is accountable for attaining budgeted visits, meeting clinical care measures, and locking records within a 72-hour window. The position emphasizes comprehensive, continuous, and coordinated care for adults and the elderly, managing both common and complex illnesses. The Internist will also focus on chronic disease management, health promotion, and the prevention of illness and disability, while providing excellent customer service and care.

  • Provide direct primary care medical services.
  • Examine patients and review laboratory, referral, and imaging results to determine diagnoses.
  • Document visits accurately using appropriate templates.
  • Attain budgeted visits and meet clinical care measures.
  • Lock records within a 72-hour window.
  • Manage both common and complex illnesses in adults and the elderly.
  • Utilize chronic disease management skills in a team-based, patient-centered approach.
  • Provide comprehensive health care including health promotion and maintenance.
  • Incorporate a holistic healthcare model addressing protective and risk factors.
  • Understand diagnosis and treatment of cancer, infections, and diseases affecting internal organs.
  • Provide excellent customer service and care.
  • Educate patients with appropriate materials based on their health literacy needs.
  • Participate in daily huddles and incorporate ancillary health team members in patient care.
  • Set and monitor self-management plans.
  • Assist in assurance of clinical procedures and maintenance of clinical protocols.
  • Complete chart and peer review responsibilities in a timely manner.
  • Assist with performance improvement efforts and staff education.
  • Participate in the preceptorship of mid-level practitioners.
  • Engage in community events and site/provider meetings.
  • Meet Network and credentialing expectations for licensure and employment.
  • Perform other duties as assigned.
  • Doctor of Medicine or Doctor of Osteopathic Medicine degree.
  • Licensed in the state of California as an Internist.
  • Board-eligibility or certification in Internal Medicine may be required or desired.
  • Experience in chronic disease management.
  • Familiarity with holistic healthcare models.
  • Skills in creating complex documents in Microsoft Word.
  • Advanced functions of Microsoft Excel for database management.
  • Ability to develop presentations in Microsoft PowerPoint.
