The organization is seeking a Board-Certified or Board-Eligible Internal Medicine Physician to join its expanding adult primary care team in the New Haven area. This full-time, permanent role is predominantly outpatient, supported by an experienced nursing and administrative team. Requirements
- MD or DO from an accredited institution
- Board Certified/Board Eligible in Internal Medicine
- Eligible for Connecticut licensure
- Completion of an ACGME-accredited Internal Medicine residency
Responsibilities
- Provide comprehensive outpatient adult primary care
- Manage chronic conditions such as hypertension, diabetes, COPD, and heart disease
- Perform routine preventive care, annual exams, and wellness visits
- Coordinate care with specialists, behavioral health providers, and allied staff
- Utilize EMR effectively and maintain accurate, timely documentation
- Participate in quality improvement initiatives and team-based case discussions
- Stay current with clinical guidelines and internal medicine best practices
