Key Features of our Primary Care Model: After-Hours Call Program: Physicians are not responsible for after-hours call coverage. Virtual Clinic Support (VCS): A centralized team of advanced practice providers work remotely to manage clinical tasks that traditionally fall to primary care physicians, including managing clinical inboxes, test results, patient messages, medication refills and routine patient follow-up. Team-Based Approach to Care: Our interdisciplinary team includes Social Work, Behavioral Health specialists, and a dedicated Medical Assistant to ensure comprehensive patient support.
