About The Position

As a Clinical Pharmacist, you will be a vital member of the multidisciplinary care team, delivering comprehensive medication management across a broad range of acute and chronic conditions. Your role will include optimizing pharmacotherapy, supporting transitions of care, contributing to antimicrobial stewardship efforts, and ensuring the safe and effective use of medications. Requirements

  • Graduate of an ACPE accredited College of Pharmacy
  • B.S. degree in Pharmacy Doctor of Pharmacy (PharmD) degree; PharmD preferred.
  • Current licensure as a Pharmacist in practicing state.
  • Completion of ASHP accredited PGY-1 Pharmacy Residency or 3 years clinical pharmacy experience.
  • Knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments

Responsibilities

  • Communicates effectively with healthcare providers and caregivers to ensure continuity of pharmaceutical care across shifts and teams.
  • Collaborates within multidisciplinary teams to provide patient-centered care, including identifying high-risk patients and implementing safety interventions.
  • Participates in medical emergencies and provides discharge medication review, reconciliation, and counseling.
  • Delivers direct patient care through decentralized and service-based pharmacy programs.
  • Provides real-time medication management in emergent and high-complexity clinical scenarios.
  • Facilitates acute interventions including medication procurement, preparation, and therapeutic monitoring.
  • Makes evidence-based, patient-centered medication recommendations and provides timely drug information.
  • Provides accurate, safe, and timely medication therapy tailored to patient age and needs.
  • Conducts critical patient monitoring and reviews profiles to prevent or mitigate drug-related problems.
  • Participates in pharmacy operations, including central and satellite dispensing, and specialty areas.
  • Oversees technician activities and pharmacy operations related to procurement, ordering, and dispensing of specialty medications (e.g., chemotherapy, parenteral nutrition, controlled substances).
  • Maintains proficiency in hospital systems and medication ordering platforms.
  • Leads assigned patient service lines, clinical areas, and therapeutic programs.
  • Designs and implements stewardship and medication restriction/surveillance programs.
  • Tracks and evaluates pharmacy programs for operational, quality, and financial efficiency, benchmarking against best practices.
  • Develops and oversees policies for drug purchasing, usage, distribution, and control.
  • Precepts pharmacy students and residents; contributes to ongoing education of clinical staff.
  • Facilitates experiential education and mentorship for students, residents, and staff.
  • Develops training experiences and competencies and fosters new teaching relationships.
  • Maintains proficiency in preceptor roles and actively contributes to education and mentorship.
  • Identifies and implements improvements in the medication-use system to enhance safety, outcomes, and cost-effectiveness.
  • Participates in emergency medicine-focused research, quality improvement, and practice advancement initiatives.
  • Initiates clinical research, quality improvement projects, and provider education to advance pharmacy practice.
